Medically Home grows home hospital care

Medically Home has announced that more than 45,000 patients are supported on its acute hospital care at home platform. More than 20 health systems in the US employ the company’s platform. Over half of all patients receiving hospital at home care under Medicare and Medicaid are enrolled on the Medically Home platform.

Medically Home’s model provides care coordination, biometric monitoring, logistics and a fulfilment platform that allows qualified patients to receive hospital care in their homes.

The Medically Home technology platform integrates with hospitals’ EHR via bi-directional data interfaces. This makes it possible to deliver hospital care in the home using a differentiated network of rapid response service providers that enable health systems to extend their brand of hospital care to patients in their homes.
